Identifying the Problem

Community healthcare organizations need strategic plans that connect long-term aspirations with the realities of delivering care. In a time of fiscal and operational pressure, strategy is what creates alignment, within an organization and with the communities it serves. With no shortage of opportunities competing for attention, focus and clarity are what keep an organization pursuing the right ones: those aligned with its ambitions and its communities’ evolving needs.

Taking Action

Developed a clear, forward-looking three-year strategic plan and supporting action plan in partnership with the organization’s leadership and Board. Conducted a detailed environmental scan and engaged internal and external stakeholders to align people and activities with one another and with the organization’s vision, mission, and values.

Results that Speak for Themselves

  • Clear three-year strategic direction aligned with the organization’s vision, mission, and values.
  • Strong organizational buy-in supported by community perspectives and operational realities.
  • Confidence in an achievable plan with a supporting action plan to guide implementation.

How We Got There

The Challenge

A non-profit home and community care organization wanted a clear, actionable strategy to guide its next three years and strengthen its ability to help people live independently for as long as possible. The plan needed to look ahead while reflecting the organization’s operational realities and the communities it served.

The challenge was to bring diverse perspectives into a shared strategic direction that people across the organization and community could support, understand, and translate into action.

The Solution

Optimus SBR conducted a detailed environmental scan to understand the organization’s strategic environment and inform the development of its plan. Engagement with clients, families, staff, partners, and leaders added community perspectives and operational insights, ensuring the strategy reflected the experiences of those receiving, delivering, and supporting care.

The team applied its Accountability Framework System TM (AFS) strategic planning methodology, which focuses on aligning people and activities with one another and with the organization’s vision, mission, and values. This approach kept the connection between purpose and execution central to the work, ensuring that the proposed direction was meaningful to the organization and practical to pursue.

The resulting three-year strategic plan established a clear direction, while the supporting action plan connected that direction to implementation. Together, they provided a forward-looking, action-oriented approach grounded in the organization’s operating context.

The Results

The organization gained a three-year strategic plan and supporting action plan with strong organizational buy-in and confidence that the plans were practical and achievable. Reflecting community voices and operational realities helped establish a shared direction that people across the organization could support.

Together, the plans provided a practical foundation for moving from strategic intent to action, positioning the organization to advance its commitment to helping people live independently for as long as possible.
